Will my insurance cover my stay?

Medicare and Medicare/Managed Care insurance cover an eligible patient’s stay for a defined period of time. For those who qualify, Medicaid covers the cost of long-term care. Patients, who require stays not covered by insurance, may pay privately.

What are the steps for admission to a long or short-term care facility?

Prior to an applicant’s consideration for admission, a New York State assessment form (PRI and Screen) must be completed by a registered nurse. These forms are reviewed by our admissions team to determine eligibility for care. Our finance staff then helps determine the applicant’s payment source(s). Upon availability of an appropriate bed, the eligible patient is admitted.

  • A hospital – the hospital discharge planner can fax the PRI and Screen forms to our Admissions Office for evaluation at (631) 539-3695.
  • Home – a PRI and Screen forms may be completed by one of our Home Care nurses or a visiting nurse and faxed to our Admissions Office at (631) 539-3695.
  • Another nursing home – you may request that the social work department fax the PRI and Screen forms to our Admissions Office at (631) 539-3695.
